Filling vs crown: which teeth need which?

  • Incisors and canines (front teeth). Unless the tooth was badly broken in an accident or already had a crown, a front tooth usually does not need a crown after a root canal. The dentist seals the back of the tooth with a composite filling (€120 to €210 if not included), and can use internal bleaching if the tooth darkens over time.
  • Premolars and molars (back teeth). Back teeth take up to four times the biting force of front teeth. Once the roof of the pulp chamber is opened for a root canal, the side walls (cusps) can split apart under a hard bite unless a crown (€800 to €1,200) or full-coverage ceramic onlay holds them together.
  • Post and core (€150 to €300). If decay has removed most of the tooth above the gum, the dentist cements a glass-fibre or metal post into one of the root canals to anchor the core build-up that the crown sits on.

Zirconia, E.max ceramic and porcelain-fused-to-metal

Irish dental clinics mainly fit three types of crown after a root canal, all in the €800 to €1,200 range:

  • Monolithic zirconia. Milled from solid ceramic block — the strongest option for back molars and people who clench or grind their teeth, with no dark metal line at the gum.
  • Lithium disilicate (E.max) all-ceramic. More translucent than zirconia, making it the top pick for visible front teeth and upper premolars.
  • Porcelain-fused-to-metal (PFM). A metal thimble covered in tooth-coloured porcelain; strong and reliable, though a thin grey margin can show if the gum recedes over the years.

Questions patients ask

Are both the root canal and the crown eligible for 20% tax relief?

Yes. Revenue lists both endodontic treatment (root canal) and crowns as non-routine dental treatments qualifying for 20% tax relief on Form Med 2, even if a specialist does the root canal and your own dentist fits the crown.

What happens if I delay getting the crown after a molar root canal?

The temporary or core filling can wear down, leak saliva back into the root filling, or allow one of the tooth's outer walls to crack below the gumline. If the root splits vertically, the tooth has to be extracted.
